Nigerian man massaging around his knee with both thumbs while seated at home

For a painful knee, a massage cream or balm rubbed into the joint and the thigh muscle above it is the usual choice. Warming products suit stiffness; cooling ones suit a knee that feels hot and irritated after activity.

How you rub matters as much as what you rub. Three to five minutes of proper massage beats a quick smear. And a knee that is swollen, locking or giving way should be seen by a doctor rather than treated at home.

The question gets asked exactly like this, and it deserves a straight answer. But the honest version has two parts: what to put on it, and what to actually do with your hands once it is on. The second part is where most people lose the benefit.

What people commonly use

Option Sensation Suits Worth knowing
Massage cream Neutral to warm Daily use, proper massage Stays workable long enough to rub properly
Cooling gel Cool After activity, hot weather Absorbs fast, so less good for long massage
Warming balm Warm Morning stiffness, cool weather Can irritate sensitive skin; test a small area
Plain massage oil Neutral Anyone reacting badly to active ingredients The massage does the work here
Warm compress Warm Stiffness before movement Costs nothing and often helps as much
Warm cloth held against a knee to ease stiffness

How to actually massage a knee

Most people apply cream to the kneecap, rub for fifteen seconds, and stop. The kneecap is bone with skin over it. There is little there to massage.

  1. Start above the knee, not on it. Spend a minute on the thigh muscle, working upward towards the hip. This muscle pulls directly on the knee, and it is usually tighter than the joint itself.
  2. Work around the kneecap, not over it. Use your thumbs in small circles along the edges, where the soft tissue is. Both sides, top and bottom.
  3. Do the inner side carefully. The inside of the knee is the most common sore spot. Firm, slow circles, without forcing through sharp pain.
  4. Go down to the calf. The calf muscle attaches behind the knee and pulls on it too. Another minute here is worth the time.
  5. Finish by bending and straightening. Ten slow, unforced bends after the massage, to let the joint move through warmed tissue.

The whole routine takes about five minutes. Doing it once properly does more than applying cream four times in a hurry.

Warm or cool for a knee?

Stiff knee, worse in the morning, better once you move: warmth. It loosens the tissue before you ask the joint to work.

Knee that feels hot and irritated after a long walk or unusual activity: cool. And if it is visibly swollen, that is a reason to have it examined rather than to keep rubbing it.

Do not rub. See a doctor.

The knee is hot, red and visibly swollen. It locks and will not straighten. It gives way when you put weight on it. You cannot bear weight at all. The pain began after a fall or a twisting injury. There is broken skin, a wound or a rash over the area.

Massage on an inflamed or injured joint can make things worse. None of the products above are designed for these situations.

What not to do

Do not massage through sharp pain. Discomfort that eases as you work is fine. Pain that spikes is a signal to stop.

Do not apply anything to broken skin, and keep all of these away from the eyes. Wash your hands afterwards, especially with warming products.

Do not wrap a warming product under a tight bandage unless the label says you can, since covering it can intensify the effect on the skin more than intended.

Common questions

What can I rub on my knee for pain?

A massage cream, balm or gel applied to the knee and the thigh muscle above it. Spend three to five minutes massaging rather than just spreading it on.

How many times a day should I apply it?

Follow the frequency on the label, commonly two or three times daily. More often is not better and can irritate the skin.

Can I massage a swollen knee?

No. A swollen, warm knee should be examined by a doctor. Massage on an inflamed joint can worsen it.

Does hot water help knee pain?

A warm shower or warm compress helps a stiff knee, especially before movement. Avoid heat on a joint that is swollen and hot.

Is it better to rub before or after walking?

Before, if the knee is stiff and you want it looser. After, if the knee tends to ache once you have finished. Some people do both, within the label’s stated frequency.
